H1N1 Flu Vaccine Permission Form.

The Cincinnati Health Department has notified us that St. Teresa family members will be able to receive the H1N1 Flu Vaccine at the H1N1 (Flu Vaccine Event) here at St. Teresa.  We do not know the date or time yet. 

Priority will be given to:
      1.     Pregnant women
      2.     Siblings between  5 months and age 5
      3.     Family members between ages 18 and 64 who have  chronic illnesses such as Diabetes, Asthma, Cancer etc…
